Forms

Create a form for a board, choose the questions and share the link: every submission becomes an item.

A form lets other people submit a request or report something, even without an account. A form is a view of a board. Every question is a column of that board, and every submission becomes a new item.

Create a form

  1. Open the board.
  2. Next to the views, click + (Add view) and choose Form.
  3. A form is ready right away, with the board name as its title. Only the question for the item name is on it.

At the top you switch between three modes: Results, Edit form and Share form. A viewer only sees the results.

Choose the questions

In Edit form, the Content list on the left shows all columns of the board.

  1. Put a column on the form with the eye icon. Click it again to take it off.
  2. Drag the questions into the order you want.
  3. Click a question to set up the Question on the right: its own title, a Description, Required, and for a status or dropdown column Which options people see.
  4. Use Content to add a Text block (for example a note) or a Page, so the form spans several screens.
  5. Save your changes with Save to view.

Some columns cannot be a question, such as a formula, a button, time tracking or a connection. They are greyed out; hover over one to see why. The Description question becomes the first update on the new item, with any attachments.

Under Settings you choose, among other things, the title and description, a welcome page, One question per screen, the group responses go to, and what happens after submitting: the thank-you page or Redirect to a link. Under Design you pick an Accent color and can hide the Qyveno logo.

Required columns

If a column of the board is required, it is always on the form as a required question. You cannot remove it. That way every item from the form gets that value too. If a required column cannot be a question, the editor warns you that the form creates items without that value.

Open Share form.

  1. Under Link to the form, click Copy and send the link around, or put it on your website.
  2. The Form is open switch opens or closes the form. A closed form tells people it is closed.
  3. Did the link end up in the wrong place? Choose Create a new link. The old link stops working.

Opening the form and creating a new link need the Share forms publicly permission. An admin sets that under Administration › Permissions.

For everyone or members only

By default anyone with the link can fill in the form, without an account. If you turn on Only signed-in members under Settings, people who open the link first sign in with an account of your organization.

A people column can only be a question on a members-only form. On an open form, anyone would otherwise see the names of your colleagues.

What happens with a submission

  • Every submission becomes an item in the chosen group, by default the first group of the board.
  • Automations that run when an item is created also run on a submission. That way you can assign someone or send an email.
  • On an open form, "Form" is shown as the creator of the item. When a signed-in member fills it in, their name is shown.
  • Results shows only the items that came in through this form.
